Upload documents to create custom form?

Hello all

I need some assistance please. I need to create 10 different form for online consultations. I was wondering if there is a way to simply upload a document to create a form, that once filled in online, one would be able to submit with a button?

I would prefer to not have to create a custom form for each consultation form that I need.

Is this possible?
